I would like to know if in your organization feedback meetings take place ? meeting with team to evaluate team lead and manager of the project. If yes, then whats the frequency ?

Ideally, in a matrix organized project, the PM gives formal feedback to the functional managers and the team gives feedback to the PMO (or to whomever the PM reports).
Informally, the team should be critiquing itself and making improvements from within when possible. For "agile" teams, this can be during the sprint Retrospective. For longer projects, it's important to have these meetings more often, certainly at the end of each phase, as personnel may come and go from the project. For long waterfall projects, the designers and architects may be through with their work months before the project comes to a conclusion. Saving Changes...
