Version versus Revision control of a document

Hi I have been grappling with this for some time. A document gets revised many times and there could be subsequent versions of the same document. I am interested to know what procedures go into managing revisions of documents and if this is the same as a version. Our current practice is to use versions for updates to documents thru its life. Wonder if this is the right process
