Change Lifecycle Framework Model.

Hello,
In "Managing Change in Organizations: A Practise Guide" (PMI Global Standard) , Page 20 Fig. 2-5 depicts a Model called the "Change Lifecycle Framework". Does anybody have any experience or information about working through the stages of that Model.

I would like to use this model for managing a change and would appreciate any experience or guidance in relation to this Model.
