What approach should be taken when creating a PMO charter? Any recommendations regarding format as well as what areas/topics should be covered and to what level of detail? Appreciate any/all feedback.

I can show you mine if it helps, but as has been said there is no "one size fits all" to this. It seems to be a balance between what the organisation needs and what it will accept.

The change formula f(D,V,F)> R...where D= level of dissatisfaction, V= The vision of where you want to be, F= the clarity of the first steps to be taken and R= the resistance to change...is very pertinent. Check the levels of dissatisfaction and resistance, formulate a vision that addresses them and make the first steps clear and you will be on yoru way you "your" PMO.
