Executive Management survey on value of PM

We are in the process of developing a survey to elicit from executive management and our PM community a. what PM processes they place the most value on and b. their vision of what a PM based organization is like. Our intent is to focus on the future and judge relative importance of PM practices to our stakeholders. Additionally, we see this survey as shaping the direction of our future PM implementation activities. Any ideas?
