What model/framework do you follow in your project for change? Could you please share/advise on strategy in the process of leading change? Do you use any change adoption tools?

There should be a change management tool to document, decide and manage the life cycle for a change. We use Bugzilla and vTiger for one customer and having our own tool called ATS. This gives us lot of attributes for a change.

A model/framework I have found useful and practical is ADKAR from Prosci. ADKAR is an acronym (of course!) for awareness, desire, knowledge, ability, reinforce.
What I like is that it starts with the basic framework, or process, and it can be scaled from there. There are also many supporting tools for the framework that are available.
The downside is that ADKAR is proprietary, but, it is still a good model.

We developed a multi-dimensional change model in our book Chasing Change, by Wiley & Sons, 2009, which consisted of three dimensions: Physical dimension (infrastructure, technology), Organizational Mindset dimension (processes, culture), and the Personal Mindset dimension (what's in it for me and how the individual succeeds). Each of these dimensions is driven by change demands from the external and internal environment. We provide a robust assessment tool in the book which assists in measuring 13 capabilities of the change-ready organization, which may be helpful.
