A model/framework I have found useful and practical is ADKAR from Prosci. ADKAR is an acronym (of course!) for awareness, desire, knowledge, ability, reinforce.
What I like is that it starts with the basic framework, or process, and it can be scaled from there. There are also many supporting tools for the framework that are available.
The downside is that ADKAR is proprietary, but, it is still a good model. Saving Changes...

We developed a multi-dimensional change model in our book Chasing Change, by Wiley & Sons, 2009, which consisted of three dimensions: Physical dimension (infrastructure, technology), Organizational Mindset dimension (processes, culture), and the Personal Mindset dimension (what's in it for me and how the individual succeeds).
