Knowledge management plan and template

Can somebody please share the knowledgement management plan and template to [email protected] if you have implemented .

i am not really sure what you are talking about?!?!
Knowledge management is just a complete discipline on its own, just like project management i would like to say.
So, there is - from my perspective - not that specific "knowledge management pan/template".
Or do we talking about communication here ... ?
If not and you want to perform real knowledgemanagement within your project(s), i guess you have to develop your own process therefore which is related to your organization and to the performing people.
Doing so you can't avoid to develop your very own template therfore depending on which KM system you will use.
KM with just a plan excel, word file whatever ...will not be succesfull ...that's for sure.

Just my 2 cents ...
