Entropy in Project Management: What have been your experiences when dealing with this Issue and how have you manage it in your projects?

Entropy is something that will be there in any project and presents an interesting dilemma. The traditional approach to Project Management suggests that we should try to deal with it by using Risk Management strategies, while the newer approaches suggest that we should try to deal with it by being able to react to changes faster.
