Hi there,
As a member of PMO structure in financial structure, we have difficulties to make project managers to provide project estimate to complete (ETC).
To understand why, we are thinking to establish a simple project management maturity model.
The objective is to bring explanations, set up actions plan and to help those project managers.
I was thinking about Project manager skills, project complexity and organization process as analysis axis for the maturity model.

Project conditions changes day by day so ETC also changes. Most of time PM is held accountable by higher managers and changing ETC is considered as non performance of PM. May be such unreasonable environment might be restricting your PM's to give ETC. PM is a person who manage balance in cost, quality and time within given circumferences, he may or may not achieve absolute performance parameters due to changing conditions not in control of PM. I agree with you, pressure for top management can contribute to the unwillingness of project manager to communicate about ETC. so as you suggesting it's better to add "external factor" impact (e.g. scope change frequency) on project as analysis axis. When I am managing a 6-12 month project, I do ETC's Weekly. When I look at the schedule of tasks that need to be completed at the end of each week, I ask my resources (i know they are people) how much longer they need. I update those ETC's in my spreadsheet, and then write a change order for the customer for the additional or reduced number of hours.
Reporting back to the customer is the most important part of the ETC process as they are the most impacted. Saving Changes...
