Please don't dismiss Rami's answer too quickly, because there is more value in creating your own spreadsheet (or similar template) for EVM. When you lay out all the EVM formulas, you'll notice that the variables are rather circular. You have to collect project data to start calculating ratios, and your project data might not exactly fit into a generic template.
Not only that, but some numbers can be generated by multiple methods, depending on the data available or the needs of your project. For example, the PMBOK gives 4 distinct methods to calculate EAC. You'll have to decide which method gives you the most accurate and useful numbers.
Bottom line: although it could save you considerable time if it works, a ready-made template might not necessarily be the best solution for your project.
