If I may add to that list,

17) Gross impact and weighted value

In addition to what has been already listed, consider also drafting a (brief) risk management plan in which specify the criteria for risk probability and impact, how often review the risk register, etc.
- Creation Date / Modification Date - Usually this is a living document, from my point of view is important to record the date that risks are identified or modified
