2- Risk Issue
3- Category
4- Title
5- Description
6- Response Strategy
7- Risk Responce
8- Comment/Result
9- Due Date
10- Risk Owner
11- Status
12- Probability of Occurence
13- Impact
14- Dependancy

For sure you can adapt/remove part of them based on project complexity

In addition to what has been already listed, consider also drafting a (brief) risk management plan in which specify the criteria for risk probability and impact, how often review the risk register, etc.

16) Severity
17) Gross impact and weighted value
