Readiness Doc Input Needed

I've been tasked with putting together a Readiness Document / Process...basically..what should be done when determining if the product or solution we are delivering is ready to be implemented. I'm thinking there should be go / no go decisions at certain intervals depending on amount of risk, users impacted, etc. All stakeholders should have a chance to have a say / vote / raise questions & issues etc. Does anyone know of a template already out there so I don't need to reinvent one? Thanks much!
