Interesting question. I am not sure there is one biggest problem. Some problems I have faced are:
Lack of alignment of the project/program to organizational strategy (pet projects)
Lack of appreciation of the true complexity and work required to complete the program
Resources who are not fully committed to the program but are working part-time or are "starting soon"
Unrealistic expectations
Employees who are disengaged
Too many "priority projects" taking place at once
I think that the new focus on business value will be very helpful. Once projects are evaluated based on business value, it will be easier to understand how and whether projects are aligned with the strategic organizational goals, it will be easier to prioritize projects against each other and to ensure that resources are working on the priority projects.
But all that is moot until engagement is higher among employees. With high engagement much more is possible!

- Engaging team members when the execution part is very routinary.

1. People's resistance to change is biggest issue. People strangely quote like--'If my old Mercedes can work fine why should I buy a New BMW' when encountered problems in accepting the changes implemented by new systems.
2. Managing expectations is another big challenge as it is purely psychological issue and almost unique in every project.
3. Availability of qualified resources and their turnover on the project, scarce availability of resources well trained in the emerging technologies is a big constraint on the project timelines and quality. Saving Changes...
