From my experience, some of the mistakes are noted here.
1. Miscommunication
2. Lack of co-ordination between multiple organizations or even stakeholders.
3. Lack of understanding of the final objective and cooperation between multiple disciplines

I'll echo Sergio's response. A number of the ideas presented by others are symptoms of the root cause which is the expectation gap related to either the desired business outcomes or the cost, schedule, quality or other requirements to achieve those outcomes.
There is not the only one biggest problem in the project. Conflicts/ Problems are an inherent part of a project and cannot be avoided and the importance/ priority change over time during the project. In this context I follow the argumentation of Sergio Luis Conte's response. Saving Changes...
