Who initiates and is responsible for the project charter?

Is your organization's Executive Sponsor initiating the project charter? If yes, how did the organization communicate and adopt this policy?

According to the PMBOK guide the project charter is the document issued by the project initiator or sponsor that formally authorizes the existence of a project and provides the project manager the with the authority to apply organizational resources to project activities. The project charter should be authored by the sponsoring entity. The project manager participates in the development of the project charter.

In my company, the Business Case & Project Charter is prepared by the PM in collaboration with the Project Sponsor but it is formally authorized by the Project Sponsor.

Doesn't that reduce the buy in and commitment of the project sponsor?

On the contrary, it does ensure that he is fully committed as both the PM and Sponsor prepare it together and the Sponsor is the one who formally authorizes it so I am not sure how this would reduce the buy in and commitment.
