I am looking for one page (A4) feedback form for the purpose of employees feedback among colleagues. Please share if you have one.

It is my idea to introduce one page annual feedback form at my workplace. Hence requesting you all to pour your thoughts.

You might be interested in this. The idea of the feedforward interview/review where the focus is on goals and not standards aligns with the work of Boyatzis & Smith that encourages coaching with compassion rather than coaching for compliance (which is generally what is on offer in the workplace).
The questions in this article are more geared to the person being reviewed, but you could reword them for others to provide a 360 view. They would focus on their experience of the person being reviewed.

I am curious, why A4? You always can manage the template and change the setup. Maybe you are not collecting feedback from American people that are using Letter format.
