WPR are the [visual] representation of WPD/WPI. WPD/WPI is used in the M&C process group and for Integrated Change Control.

"The Work Performance Data are the initial measurements and details about activities gathering during the project work. This Work Performance data is analyzed to make sure it confirms to the project management plan and is assessed to what it means for the project as a whole... this result is Work Performance Information. This information is then organized into Work Performance Reports which are distributed to various stakeholders who need to receive and possibly act and make decisions based on the information. The reports are outputs in monitoring and control project work." - Rita Mulcahy's book on PMP Exam Prep

I'm not confused by these at all. Data to me represents the past-something had to happen to be measured, analyzed, quantified, etc for it to have been created. The data then comprises the WPI, and where do we keep all this information? In the WPR.

So do you make decisions based on the Informatin or on the reports?
I had a sample question like this and the right answer was Information:
e.g.
"What is collected from the various controlling proceses and gives sound foundation for project decisions"?
a) WPD b) WPI c) WPR d) Project documents

Demetrius` post states:
This information is then organized into Work Performance Reports which are distributed to various stakeholders who need to receive and possibly act and make decisions based on the information.

Based on the definition in PMBOK, stakeholders can make decisions with WPI and WPR. But WPI can be produced from the various processes in M&C Process Group while WPR can be made only from the Process of Monitoring and Controlling Project Work in M&C Process Group. So correct answer might be WPI.
