I think it is very difficult to avoid conflicts in an organization, when we work with different cultures these increase and sometimes we get out of hand.

As Aaron commented above, it might be a matter of dealing with. Conflicts can be viewed as opportunities to have when we are overcoming or using them favorably. Let's assume that there are serious meetings in your project but with no conflicts or arguments. Can we expect that the effective discussions or decisions can be made with just participants holding back their different opinions or disagreements?

I approach conflict the same way I approach concerns presented by our markets about whether or not there will be issues following a large software implementation. The real concern is not whether or not it is going to happen, it's about how well we prepare for it to happen and how we deal with it when it happens. There will be issues post-implementation. There will be conflicts. How you handle them says more about you than whether or not they happen.
Thanks Aaron

You can mitigate them but you can avoid them. It is inherent to human being.

I agree with most of the comments above.

I would approach this in a planned way by starting from the Organizational Assets where I (or others) would already have documented various types of conflicts related to differences in perceptions based on background, skill area, working style, culture, personal, etc.

Identifying the base for the conflict is important and then finding a common ground. Again we'll go with the time tested formula : Identify -- Avoid -- Mitigate -- Compromise.

I hope that helps. Thank you !
