Does anyone out there have a good template for a 'Change Control Form'? Or a best practices list?

Hi Alan,
I recently put together a quick change control process for a project that I'm working on. There are a huge number of CCM templates available both on this site as well as other PM sites. However, the process that I put together was a simple Change Control Form using Word which I send out to any person requesting some type of change to the scope of the project such as a new requirement. To keep track of the forms, I built a change control log using Excel that I hyperlinked to each form. The log lists general info such as Change name, description, date opened, status, etc. Obviously, the best method of handling change control is w/ some type of application or at least an Access database that is tailored to CCM. However, if you don't have anything like that handy, this is a quick and fairly painless method of logging your project changes.
