Does anyone out there have a good template for a 'Change Control Form'? Or a best practices list?

Ed, I have forwarded a MS Word and MS Excel version to your email. I prefer the Excel simply because it totals the dollar impact of all the changes. Hope this helps.

Does anyone have a good template for a 'Change Control Form' acceptable in a Pharma industry as per the guidelines and whether we can have a single standard format for Project change, process change and design change. ?

Hi

The best standards I've seen are from IEEE. Templates are good, but effective change control procedures have to be adapted to the industry and culture of the organization. Things such as product lifecycle, customer demands, rate of defects and thye health of the application in the field are some considerations to address as you come up with your very own process.

I am attaching a template, but keep in mind that some things may not apply to your organization. If you need help you can always e-mail me on here.
