Recommended book on Change Management

I would like to learn more about (Organisational) Change Management. What book(s) do you recommend?

(Just to be clear: the topic I am looking for good books about is Organisational Change Management – not organisation for managing change in a project. I.e. not Change Authority nor Change Control Board :-)

1 - Goes deep on human behavior in organizations (social-psychology), which we should understand to comprehend why, for example, people resist change
2 - Enhances understanding of management and leadership that goes beyond change (anyone managing teams, I think, will benefit from this text)
3 - Provides extensive list of references, which opens the door to further study
