How to balance the time limit of delivery and refactoring anytime?

Agile emphasizes that each design must be just enough, and we do rafactoring anytime whe the new request comes. But many times, the changes of refactoring are subversive. It means more additional developing and testing work. Especially, the performance testing work always crosses the sprints.
