Lessons Learned

Should the lessons learned document provide observations on problem areas or should it also provide recommendations on how to prevent future mishaps? Sometimes the solution to the problem is not easy.

Dear Anonymous, many people like to see lessons learned documented along with recommendations for improvement. When a recommendation to fix a problem area is difficult to determine, an appropriate recommendation could be to convene a group to conduct an analysis of the problem and recommendation for imrpovement. I'd say it should provide everything it can - even if it is a problem that there is no clear solution to, it is valuable to future projects to know that such a problem may arise, and indeed it is valuable for them to know that if it arises it is not something with a real "fix" - so they need to allow space for alternative to cope with it.
