Good question. My three (assuming IT projects):
1. Projects delivered on time/within scope/within budget - for each project launched to production, was it delivered on time, within agreed-upon scope, and within agreed-upon budget (all taking into account any sanctioned change requests)
2. System stability first 3 months after launch - Did the PM deliver a quality solution to production or is the solution unstable and impacting ability to transact business
3. Degree which PM contributes to PM competence level of organization - Does the PM actively work to grow other PMs to help them be better or does he/she view other PMs as competition.
Notice I didn't include benefit realization. I see benefit articulation as job of the sponsor and see the sponsor as the one who must ensure the requirements contribute to benefit realization. Saving Changes...
