Process Improvement maps & steps

I would like to ask the following questions regarding process improvement.
1) If within a certain process a proposal for example was sent to a customer. The next activity on this process would happen when the customer signs the proposal and send it back. Would this activity and the following ones separated in a separate process or the process will remain active till the customer send the signed proposal?
2) How does the parallel activities represented in process maps?
3) What is the suitable book I can read to understand exactly how to develop and improve the processes in a software house?

For anyone looking at different requirements management tools, check out Gatherspace.com http://www.gatherspace.com.

While not as rich as other tools on the market, Gatherspace is free, online and provides some pretty cool gathering and reporting features.

Hi Michael, I was looking around on the web for ideas on improving the work process in an engineering department and came across this forum. I couldn't help but notice the demand of your book. I know it's been 2 months since you last offered it, do you have any left that can be mailed to me. If that offer had already expired, what are you charging for that book?

Hi Michael,
I have just completed a project to implement ITIL processes, and now need to look at implementing a continuous improvement framework. Does your book cover this ?
