What is the best way to provide constructive feedback to your manager or management?

Since I am new to the field of PM, I would like to know what is the best way to communicate constructive feedback up the food chain. Any thoughts or article links would be appreciated.

As mentioned feedback should also include proposal/s of how to implement a change and yes it should be done with respect. Unfortunately the respect aspect is sometimes abused. People by nature do not like to be told that what they did can be better, or different. I've come across companies where any sort of criticism is frowned upon and back patting will get you further and constructive criticism.So be aware of the culture when you do it.
