Do you have a process review for your projects, that help identify if the risk management (or other processes) in use, add risks to your project?

We did it in two steps... detailed procedures and process checklists for Project Teams use, independent project reviews by PMO... in both steps we look for gaps and risks.

I see many organisation that want to do risk management but don't plan time for it!

Most organisations have a lessons learned workshop and/or Post Implementation Review. The process focuses on the program and/or project and/or phase rather than the processes applied to get there. It would be great to get the Program/Project Management office(s) looking at these processes and what they bring contributing to the overall success, having fit for purpose mitigation strategies including sufficient budget contingency.
