Program Managers - How do you create/foster true organizational and portfolio excellence?

What kind of things come to mind when you think about what really works? What could be better?

How do you close the gaps (or how have you in the past)?

Hi Haley, I see you are in Portland too:-)
Personally, these are the things that come to my mind and have voiced several times.
1. Program managers- some are nitpicky, pay attention to trivial things. That should change and let the PM do their own style on managing the projects.
2. They should be on the look out for the Program level/Portfolio level status deviation on the projects because they are the ones closer to the Portfolio Development Managers, so they have a better insight. But I noticed they rely on PM's.
3. Escalations- they should know the right time and right way to escalate quickly. Sometimes, they lose that skill and the project slips quickly.
4. Too much process for no sense, sometimes- they should voice and obtain consensus from PM's and bring the change.

For me, I kept bringing my thoughts, disagreements, reasoning etc. Some things changed, some never did. There could be more gaps, but quickly thought of these.

First, you have to define what does mean "excellence" for your organization. Is a matter inside the quality definition which is a strategical decision. Whit this in your hand then you can start working on it.

To continue on Sergio's comment, I would suggest the PMO needs to figure out what exactly it wants to be excellent at. The PMO should have a set of core competencies that cannot be delegated or outsourced.
