I learned that Fagan Inspections are invaluable for creating very high quality software. If you are working on mission-critical applications, you should do them.

In a ideal scenario, if process variations are controlled to ensure reliability & repeatability (Cpk consistently 1.66), then over a period of time the resulting product won't need any inspection.
But in practice, the systems & procedures are handled by persons with varying capability/skills/approaches/attitude and hence the products which they produce are prone to errors/defects.
The best form of inspection should bring in sound Poka-Yoke (mistake proofing), system of "Error Prevention" rather "Error Correction".

Inspection is always a neccessity and offcourse its value adding, since cost to correct later is always higher and can affect reputation
