I do not want to mention the famous topics like communication, scope creep, ....
Currently, the biggest challenge for me is, that many customers believe they just buy agile projects and everything goes smart, faster and cheaper without fully understanding what agile practises mean to their own organisation or the people going to be assigned to projects applying agile practises. There is a lot of hype and unrealistic expectations.

This is a big one that I've seen and experienced. It's incredibly frustrating when management want Agile but they're not willing to work Agile or work in an agile mindset

Most of my challenges comes from Management's refusal to make decisions in a timely manner. When they finally do make decisions the project is already underway, and I have to scramble to incorporate them. This means I have to perform detailed analyses of the impact of their decisions on the project, which is very time consuming.

This has been my biggest issue as well. Lack of resources, and those who should give me the expert judgment don't really know all the requirements or forget some from their past projects which then throws me in a loop. No matter what I do to acquire the requirements and scope up front, I can't read other people's technical minds.
