Stakeholder & Communications Management

I am doing a thesis for my Masters in Project Management at UL. The subject matter is Stakeholder and Communications Management (particularly for IT projects). I would appreciate your help in getting a questionnaire completed on the subject (short, I swear!). I would also appreciate if you could pass this to anyone else you believe would be in a position to fill out the document.
