Looking for Advice on returning to work after Disability

I have 10 years experience in voice/data/IT applications implementation.I have been out of the workforce for more than 2 years and having trouble getting past HR.

I'm sorry to hear about your struggles. What about partnering with a recruiter or consulting firm? This will remove that hurdle with someone submitting on your behalf. The typical HR vetting is taken care of by the organization's partnership with the firm representing you.

Connie, here's the skinny.
Identify what companies, positions and people you want to work with. 2-3 a day is good milage.
Draft/script a personal letter identifying what pain/pleasure they have expressed that needs help with. You can find this info in quarterly reports, association newsletters, their own PR etc.
Revap your resume, with samples of your work linked in. Your from IT show IT girl. (see Liz Ryan on Linkedin)
Walk and regular mail your interest to the person you will work under/with and or be directly reporting to.
Follow up in two days, calendar every 4-6 day cycle to follow up and have a fresh approach each time..
i.e. What you read in a publication. What you heard on an association webinar. That you went to a Federal, State, County, Corporate/non-Profit workshop and heard...
Keep informed and broaden your reach.
Don't let the BLACK hole of employment web sites and HR Police deter you. WE have history, we have resilience, we have intellectual capital that they need! Prove it!

Leverage your network to the hilt - via LinkedIn you can determine which of your close contacts work for the companies you are targeting and see if they are willing to do a "warm" introduction to the hiring manager.

Most large companies have employee referral programs in place so it can be a win-win for them to do so.

Unfortunately, HR staff and the systems they use are indundated with applications and unless you can reverse engineer the specific "secret sauce" they will want to see in a cover letter and CV to get a maximum score, you might have better luck playing the lottery...

Sometimes HR units don't like to see gaps in your work history. You may have to be be resourceful and creative in coming up with a truthful way to fill that gap in your résumé.

You can find a variety of remote PM opportunities on Flexjobs.com. Recruiters who list on that site might be more open-minded than others with whom you've dealt.
