JT, while I don't have any specific questions I can offer this. Keep the number of questions to under 10. If you are trying to determine a baseline for satisfaction base the questions on key metrics such as those associated to delivery times, communications quality, etc. Also consider collecting 3 specific pieces of data on each question.... 1) What did they expect, 2) What do they desire, 3) what have they experienced. I have an article on gantthead that goes into this approach in more detail. The result of this approach will provide you great insight into satisfaction and perceived performance GAPS.
