First of all, there is not an hybrid model of project management. You have to select a model (practices) to tollow, for example the PMI way. Second, what you choose is an approach based on a life cycle model (predictive or adaptive), a life cycle process based on that model (V, Spiral, Iterative, Incremental, etc) and if applies a method which is based on the life cycle process. All these is very important to understand to not fail. My recommendation is taking a look to the new PMBOK (if you follow the PMI way) where all these stuff is very well explained. Adding something, there are criterias that organizations have to define to select all that. For example, inside the organization where I am working today, we have five life cycles defined and we select one based on criterias like type of product to create, time to market and others.
