We document lessons learned at the end of each project. The entire team carves out and hour or two and we go through and list things that worked and things that didn't.

Reading, and re-reading these has helped tremendously. However, I believe doing it at the end of the project is not enough - I've started a notebook specifically devoted to this and jot down thoughts whenever they occur.

Simple, but invaluable.

By doing causal analysis and resolution (CAR), opportunities to improve the process may show up. Depending on the effectivity of the method used, the number of opportunities may vary at times.

CAR is usually done at the end of a certain phase or when issues arise, however, it can also be performed at an earlier phase through risk management. With this, process improvements will help mitigate or prevent the identified risk from occurring.
