Determining the cause of the rework failure and taking corrective actions is the proper procedure, in my opinion.
Disciplinary action may follow but is not for the PM to decide.

My first though is in line with @Dan (hehe). Project manager is accountable for all project activities. Quality activities are created to assure that the project creates the product/service/result as defined. Before quality activities Scope activities related to verify scope are executed. That is the moment where the stakeholders will not agree about you have created the product/service/result as defined. At the top of all these is the business analyst who is accountable for all related to product/service/result definition and acceptance. So, while the project manager must answer for the situation you stated people assigned to QA activities are the first line of fire to avoid this situations. And no matter the type of project life cycle approach/process you are using. Saving Changes...

Yes Sante I agree with you, I don't think firing would be aligned with Pmbok guide or we assume that the pm in functional organisation so he has no power to fire

@Riyadh, I assumed that @Dan comment was a joke. Because if we do not take it as a joke then the first person to fire is the project manager. To fire people does not solve any problem. In fact, it will deepen the problem. I know that. Returning to the point it is clear that quality control process is not working. That is the place where you have to check. Obviously it is most efficient to put a good quality assurance process in place to avoid rework but because you statement quality assurance is not in place.

I think the PM should review the processes and procedures (Quality Assurance) and make changes if needed so that the cause of anticipated defects could be prevented. Anyway first investigate the root cause.
