I am trying to find industry wide standards for IT projects related to the different project phases - analysis, requirements, defects, etc. Does anyone know of any?

You may have trouble finding one industry standard for phases because different companies and different divisions within companies use various methodologies, and some even make up their own. PMI's would probably be the closest you could come to a "standard", but looking at software development in particular, you have the MSDS, RUP, etc., and they all have phase nuances. Plus, I've seen different terms used for nearly all phases from book to book. You could make a survey of the dominant types of methodoligies and map those phases, though. Saving Changes...
