A hack trying to conform.... PM template needed!

I have never had any formal training in PM methodologies, everything I have learned is from internal courses and the school of experience. I am trying to get on the straight and narrow but having a little difficulty finalizing my approach. I will list my current approach and could someone please correct me if I am wrong?
1) Project kick-off
2) Requirement gathering (BA present)
3) Create Project Charter
4) Create Roles and Responsibilities
5) Create Project Plan
6) Create communication plan (is this necessary for a 3 month project?
7) close all outstanding open issues
8) Post Mortem

Facilitate with adjusting the PP and weekly status reports

I would be greatful for any feed back on this simple framework I have been using...

The one aspect I see missing involves pre-project assessment. Do these projects come to you with a business value/case already laid out? If so, then your approach more or less mirrors what I have done on some short-term projects. If not, add that in. I do wonder why you're creating a Charter after the Kick-off and gathering requirements, but "Charter" can sometimes have a variety of definitions. I would move "Create Project Charter" to the very front. Then have your sponsor sign/approve it and then you're ready for the big kick-off blast. You could also combine 4 and 5 so that the "Roles and Responsibilities" becomes part of the Project Plan. The same could hold true for the Communication plan.

You could search for "Project Management Methodologies" and adjust your list to one of those. The web crawls with methodologies. But PMI's would provide the best framework, namely:

1. Assessment (business value, charter, scope)
2. Initiation (kick-off, identify personnel)
3. Planning (Schedule, budget)
4. Management (execution of plan)
5. Closeout (wrap it up and review)

I suggest you add Risk List also and checkout Method123.Com and TenStep.Com for templates. I've attached a SW Development Plan to get you headed in the right direction.

Thanks Ed - I have incorprated your suggestions into my project plan. The business case already came through to me and this is a 2 month project. Going to be a tight timeline peoject...

Thanks Tom - Method123 has some really great documents to download. I have modified a couple of my templates after looking at this site.
