I usually use the issue register report. Which is in the report I recorded all the backlog of the existing issue during the project run. Then, there are several columns inside such as project name, what the issue, whom is the PIC, plan start & finish date, recommend action, actual date and actual action. In my view following need to be done to address Daily Issues
1. Mechanism to collate the issues
2. Prioritise Issues based on its effect on Triple constraints
3. Discuss/ Analyse Issues with all affected stakeholders
