Best Way in my knowledge is to interact with team down. Try to reach to the ground people to take actual view of busienss perception. Your filed force must be aligned with organization object and there should not be any communication gap between team and supervisors.

I usually use the issue register report. Which is in the report I recorded all the backlog of the existing issue during the project run. Then, there are several columns inside such as project name, what the issue, whom is the PIC, plan start & finish date, recommend action, actual date and actual action. So far it's effective at least it useful to remind me and other divisions.

In my view following need to be done to address Daily Issues
1. Mechanism to collate the issues
2. Prioritise Issues based on its effect on Triple constraints
3. Discuss/ Analyse Issues with all affected stakeholders
4. Plan & implement Mitigation measures to reduce the impact of the Issues
