The biggest toxicity is when leadership isn’t told the truth – just what they want to hear. It’s hard to hear the truth, as a leader, you need to create an environment where employees feel like they can tell the truth.

what level of vulnerability leader must be willing to have uncomfortable conversations.

The earliest Waring sign that a company culture is turning toxic first sign of
a breakdown of comradery in a team oriented environment.

I guess a good sign would be employee satisfaction measured in a number of ways, not the least being staff turnover rates.

Is the company culture is not in line which your belief and behaviors you must leave the company. Is like in a married couple the wife think she can change the husband behaviors after married.

A couple of signs I've noticed are:

1. A greater than normal focus on people rather than process or systems when issues occur. Usually this goes hand-in-hand with CYA behavior and finger pointing...

2. Individual empire building (a la Game of Thrones)

In addition to turn over rate, finding a responsible, a culture base on an individual, I can think of too much focus on revenue/profit.

Sometimes, the easiest way to spot is to check how toxic the people around, this is subjective. Not to mention, toxic people creates toxic culture rapidly. Again, it will completely depend on where the focus is, as Kiron spotted, important shall be the process, not the individual. Toxicity of individuals of any kind, grade or degree shall be eradicated, however diluted forms, can it be termed such way or other, exists in any context.
