Have you used Ishikawa diagram?

How often you conduct facilitation to solve problems and which is the most technique have you used ? please give an example

Apart from other techniques mentioned above I use Ishikawa diagram quite often in identifying root cause of risks though there are other ways too.

I occasionally use it when solving quality process issues to determine the causes of defects on the production line. I also used it in the past in project risk management. Using Ishikawa diagram is a powerful tool to identify risks during the planning phase of some complex projects. It helps develop a risk register by categorizing risks, linking them to their root cause for a better understanding, and ultimately establishing appropriate risk responses for those which do not need a deeper analysis.
