My question would be, "What is your most memorable failure and what learnings do you carry from the incident to ensure it never happens again?"
Most certified PMs prepare adequately for the other questions - but this forces them to be candid. It is a great type-1 rejection question -- people who have not managed any projects will not be able to come up with a meaningful answer while people who have put their noses to the grindstone for even one project should have a bunch of examples to talk about.

Thats really good question... I will ask that "what were the lessons learnt in previous projects to lead to success/failures" and " what makes you challenge to manage- wether its client or project?" Saving Changes...
Karan's question is my usual default, but my runner up is "What do you do to stay current in the profession?". This helps me understand whether a candidate is investing in themselves. I usually follow that up with "What's the last book you read and when did you read it?".
