Depending on the specific processes and systems in your company, your procurement management folks are likely to have a contract management system where claims are managed.
If not, then as you've suggested, a simple Excel spreadsheet reflecting the following columns might be a starting point:
1. Description of claim
2. Date of claim
3. Originator
4. Status of claim - e.g. open, closed
5. Financial outcome of claim - this would be the pluses & minuses
6. Claim resolution date
7. Comments/Update log
