Job interviews can highly under-represent a project manager capabilities to get a job.

How soon to be able to take time off : )

Not being genuine, conversational, honest, clear, confident, concise, pleasant, respectful. If they don't want to be around you or don't feel like you'll fit in, then it doesn't matter how much experience you have.

Asking about overall hike percentage or appraisal cycle...
Not showing good attitude towards work to be done or expectations that a company has..

Incomplete research of the Company, their culture and preparation of your marketing speech to present how you can be an asset to their organization.

A big one is anything which shows a lack of self-awareness or low EQ.

Also, not being able to answer a situational question based on valid, past experience. If a PM provides the "by the book" answer but doesn't have the experience to backup what they actually did, that's a red flag.

when asked about weaknesses, saying "i don't have any weaknesses" or giving a generic answer like "i work too much"...

Besides failing to plan his/hers arrival to the meeting, I would say lack of EQ and lack of staying cool under pressure (which should be tested during the interview)

The candidates awareness and ability to read various situations is critical. Not going numb when the storm comes, or when under pressure from stakeholders.

Why should I hire you for this PM position?

If the candidate is just saying that he/she is passionate about it, doesn't help to stand out from other candidates. They have to demonstrate the "passion" by showing results obtained from previous employers.
