Do you include the planning of a plan in the plan?
E.g. Is the ‘Planning’ aspect of a Time Management Plan included in the Time Management Plan? For a meaningful discussion, I have picked a knowledge area(Time) which does not have an ‘Executing’ process rather than Quality, for example.
A person may understand that after Initiation of a Project then follows the Planning of say ‘how to Execute’ Time Management, ‘how to Monitor and Control’ and ‘how to Close’ Time Management process or activities, which are contained in a Time Management Plan. What about ‘how to Plan’ Time Management process, is it documented in a Time Management Plan?????
There is an iterative nature within the Process Groups, especially with planning. I know what you are saying; it takes time to plan, so is this time for planning in the Time Management Plan (it's now called the Schedule Management Plan which is a better idea than using the term "Time"). The planning portion of your project still needs to be reflected in the schedule. You can break down planning under the knowledge areas if you want to (not many people do and it depends on the size of your project), and then you would have a section in the Time (Schedule) Management Plan on the estimated time and effort it takes to complete same.
Yes, absolutely. Regardless of how meta it sounds.
Schedule management plan is one of the key components of the project management plan and deserves a healthy chunk of time. This effort must be accounted for.
As the others have mentioned, yes. But it's best if you keep it clear and written at the beginning.

Project management activities are 20%-25% percent of the total project time (in average an accpeted time). Those activities includes all related to project management.
