Can you give us a little more idea of what specifically you are looking for.
Are we talking about planning / prioritising - expected benefit, planned costs, required timeline, etc? It sounds like that is what you are asking for, kind of like an annual planning and approval exercise. However it also sounds like IS is making the decisions over which projects should go ahead which doesn't sound like an annual planning exercise.

There is a quick and dirty one that you can download from projectconnections.com - It might be a good start for you but I would also mention that you (your company) sets the requirements for determining whether or not to approve a project. Some things to consider are whether the project should be done for regulatory, strategic, new market, or operational cost reduction. Also, you may find you need phase gates if you will where you periodically review whether to continue to fund the project. For example, you will may nee to approve a PM and analyst resource to determine ROI or CBA, then review the results and make a determination to continue to fund to gather requirements \ detailed cost estimates. To clarify - we have a very detailed template that the business areas fill out and then submit to IS. In IS, the PMO manager then works with the other managers to determine if we have resources and makes a decision on the project. The problem is the business areas spend so much time on completing the template that it makes it very hard to say No. I'm looking for a simplistic template that I can use to help start people thinking of the initial request for IS resources as a 4-6 hour endeavor, but at the same time, I want the business areas to still be thinking about benefits and strategic fit of their project. Not sure if this is what you are looking for but I've attached a template for a project scope document. It isn't to long to fill out and you can use it as a draft for the business to complete (shouldn't take more than an hour or so) and then work with assigned PM to refine as you move through the process. It will help the business think about the benefits and fit for their project. Technically, I suppose it should be called a project scope document but I've worked at several companies that call it the project charter. I'll also post a process document that you might be able to reuse - to outline exactly what the process shall be and use to communicate with your business folks.
