I read it once and took notes about two weeks before I took the exam.
I focused mainly on Inputs, Tools and Outputs

I read it all once, I took notes but mostly highlighted and bookmarked, I didn't memorize but tried to understand and assimilate. I re-read the parts I highlighted and the parts I didn't understand.

During my second read through, I took notes, then used that as my primary source of PMBOK material. I typed them out, printed, and stapled as a booklet. This provided a simpler format to travel with, then further fine-tune with notes and comments.

How many times did you read it before your exam? A FEW

I was able to take a "Boot Camp" preparation course, and then used sample exams to help show me where I had weaknesses. I reviewed the PMBOK chapters where I was weakest, and worked to memorize content and formulas.

By the way, the PMChallenge questions here are fairly useful for helping to identify knowledge area weaknesses.

I read the pmbok when I started my journey to write the pmp, read the RITA Mulchy and then read the pmbok again a week before my scheduled exam. Please do not memorize the ITTOS understand them, understand the concepts how they relate to each other and how all the KA work together. I passed on my first attemp with above target.
