Main Causes of Project Failure.

what do you think the main cause of project failure?

Many causes of project failure like:

Incomplete requirements
Lack of user involvement
Lack of resources
Unrealistic expectations
Lack of Executive support
Changing requirements
Lack of planning
Didn't need it any longer
Lack of IT management
Technology illiteracy
Which one you think is the most key cause?

Didn’t need it any longer is not a cause of failure nor lack of IT Management as IT can be part of a project only.

Ive seen so many projects fail due to lack of proper communication (i.e. No Effective nor Efficient Communication)

Rami, I assumed that the PM is doing a good job on communication, it is rare to see project fail because of project manager's deficiency in communication.
and if it does that's mean the failure due to lack of PM competency, which can be replaced to avoid failure

They do not know what they want... I just pulled out of a project because of that.
They thought it is nice to have and it is easy to have.

Any of the above you mentioned can be the cause of failure, but I just let it out to relax after few months of dragging.

Rami, I assumed that the PM is doing a good job on communication, it is rare to see project fail because of project manager's deficiency in communication.

I actually somehow disagree.

I did not say the project failed because of the PM’s lack of communication. I specifically said that it is due to Inefficient & Ineffective Communication in the project as a whole because the latter could cause half of the failure reasons you mentioned above like Lack of Requirements, Lack of Resources, Lack of Stakeholder Engagement snd the list goes on.

Efficient & Effective Communication is key and it is the root cause for failure on many jobs.

Yes Kevin this is a big one it's beyond the PM, I guess mostly external causes beyond PM control.

If I had to choose one out of that list then it would be associated with not handling requirements properly, since the deliverables are all based on appropriate requirements analysis. But I agree that lack of effective communication needs to be on that list.

from my side, i have chosen, changing requirements for client satisfaction, as all you know very well that will lead to frequent project irritation, from this we can conclude that failure from stakeholders communication to project manager because he wasn’t able to identify and chose appropriate channel of communication.

The main cause (and I discussed it from years with the PMI so they have changed some things related to it inside new documentation like PMBOK. Fortunately I was not the only one) is: incorrect project success factors stated.

I remember this quote that tells the importance of planning:

"Failing to plan is planning to fail - Alan Lakein"

Of all the listed causes, in my experience, I think "Lack of planning" always leads to failure.
