Hi Abhishek,

Risk matrix - used to help risk rank a risk or display risks on a heat map. Typically using impact and likelihood ratings associated.

Risk Register - a repository to collect various risks and their relevant data

Risk report - A report generated for stakeholders with pertinent information depicted / summarized.

Beyond our explanation my recommendation is going to PMI documentation (mainly glossary and lexicon) to get the definition on those terms into PMI context. At least, is what I do to get the definition from the organization I follow with all related to project management.

Risk matrix is a grid with impact and probability should include positive and negative risk.

Regiter is often in a form of list

Report would often present top risk to stakeholder with pertinent information such has mitigations

A repository in which outputs of risk management processes are recorded.

Risk Matrix
It is a matrix that is used during risk assessment to define the level of risk by considering the category of probability or likelihood against the category of consequence severity.
